Support and Assistance to individuals through Domiciliary Care and Supported Living services

Epiphany in Care aim to provide an individualised package of care and support, to individuals who use our services within their own home. Whilst doing so we endeavour to improve their quality of life by enabling them to maintain their independence, promote their ability to make informed choices, support and encourage individuals to enjoy their lives and also to play an active part in their communities.

Our service aims to offer a practical and personalised service to adults aged between 18 and 65, and older adults aged 65+. Our service is available to individuals who primarily have a learning difficulty and additional needs such as a mental health illness, dementia, sensory impairment or a physical disability.

We aim to achieve this by meeting the assessed needs of our Service Users, enabling them to achieve and maintain their chosen outcomes by working in a person centred manner, which promotes independence, choice, control and autonomy.

The services that Epiphany in Care provides are available to everyone regardless of their religion, culture, race, ethnic origin, sexuality and sexual orientation, marital status, political affiliation, disability or impairment.
